Below is the process to follow for ordering a new, recent certified original birth certificate:

Step 1: Please provide us with complete details for whom (applicant) you want to order the birth certificate. The applicant information we need includes: First Name, Middle Name, Last Name, Date of birth, Place of birth (state, city, county), gender, Father’s full name (First Name, Middle Name, Last Name), Mother’s full name maiden (First Name, Middle Name, Maiden last name), and whether the person (applicant) is still living or not. If you have an existing old birth certificate copy, please fax it to us. We will get in touch with you if we need any additional information. We need to know your relationship to the applicant.

Step 2: Upon receipt of all the above information, we will fax, email scan, or mail you a copy of an application form. We will provide you with instructions on how to fill-up this form. You need to sign this form after all information is filled up and get it notarized locally. We will also send you a special power of attorney form duly filled in that you need to sign, and notarize locally where ever you are at.

Step 3: Once notarized, you need to send it all back to us via email scan, fax, or mail.

Step 4: We will procure the birth certificate apprising you of the progress, and if any other information needed.

Step 5: It usually takes about 4-5 business days to procure the birth certificate from the time we receive your notarized application. Upon completion, we will mail back all the documents to your specified address.
